If you are Cheerleading Teams/Crews, please:

1. Submit the Team/Crew Name.

2. Group members brief information.
2. 隊員簡單資料：姓名、年齡、表演經驗等。

3. No. of Dance and Songs with names.
3. 舞蹈及歌曲名稱、數目。

4. The period of time in performance.
4. 表演時間長度。

5. The payment you are asking for.
5. 要求報酬。

6. Finally, the correspondent Name and contact number.
6. 最後是聯絡人資料連聯絡電話。
